What you will need: 

  1. U.S. Passport – You must provide your signed passport.  Your passport must be valid for at least six months and have a blank page available.
  2. Liberia Visa Application Form – Please provide a completed visa application form in duplicate; click here for a .pdf of the application.  Because the application changes on occasion, we’ve also provided a link to the Liberia Embassy website so you may get the most recent version directly from the Embassy website.  The application form must be filled out completely and have answers for all fields.  It must display your full name as appears in your passport, and it must be signed.
  3. Washington Express Visas Visa Request Form – filled out completely.
  4. Photographs – Please provide two color, passport size photos.  The photographs must be printed on photo paper,  must be 2″ by 2″ and have a white background, and must have been taken in the last 6 months.
  5. An international Inoculation Certificate against yellow fever.
  6. Copy of your airline ticket or travel agent itinerary.

  6. An international Inoculation Certificate against yellow fever.
  7. Copy of your airline ticket or travel agent itinerary.

Vaccination against yellow fever is required.

Exempt from Yellow Fever vaccination:  children under 1 year of age.

Recommended:

  • Malaria prophylaxis. Malaria risk – mainly in the malignant (P. falciparum) form – exists throughout the year in the whole country.  P. Falciparum resistant to chloroquine and sulfadoxine-pyrimethamine reported. Recommended prevention:IV.

Vaccinations:

Certain countries may require vaccinations if your prior point of departure is from a country that is deemed a health risk.  You can go to http://wwwnc.cdc.gov/travel/content/vaccinations.aspx for current health requirements for each segment of your travel.

  1. Passport required.  Passport and/or passport replacing documents must be valid on arrival.
  2. Visa Required.
  3. Visa Issuance.  Holders of pre-arranged visa can obtain a visa on arrival, provided:
  • commencing journey in a country without diplomatic representation of Liberia; and
  • visa has been pre-arranged and paid for by local sponsor in Liberia; and
  • transporting carrier in Monrovia is informed of the following passenger’s details: name, nationality, document number, flight number, date of arrival and address of stay in Liberia.  Note: Passenger should not be boarded unless telex confirmation from airline’s station manager Monrovia is stapled to the ticket.

Warning:  Non-compliance with visa requirements will result in considerable fines.
